Two Legend Senior Living communities in Jacksonville attain zero deficiency-free surveys

The Windsor at San Pablo and The Windsor at Ortega, both Legend Senior Living® residences, have achieved a deficiency-free survey from the Agency for Health Care Administration (AHCA). The two residences joined The Windsor of Ocala and The Windsor of Venice in achieving zero deficiencies in their annual re-licensure surveys.

“Our goal for each residence is always zero deficiencies,” said Gwen Thibault, Divisional Vice President for Legend Senior Living. “This speaks volumes to the staff’s commitment to the residence and the residents we serve. This accomplishment gives residents and their families peace of mind knowing the residence is operating at the highest standard of quality service, environment and care.”

Each senior living residence in Florida must pass an unannounced, re-licensure survey by the AHCA every two years. Surveyors conduct interviews with staff, residents and families. Feedback during the interview process drive the outcomes of the survey and the potential areas that would require further review.

The AHCA survey measures regulations outlined in rule 58A.5, which include but are not limited to resident care standards, Medication practices, staffing standards, staff training, food service standards, fiscal standards, physical plant standards, incident reporting and emergency management.

Depending on severity and scope, deficiencies have different consequences for the community. Class I tags are the most severe and can result in a moratorium on move ins and/or potential license loss. Class II tags come with the potential of a fine, but will not affect the ability to operate. Class III are common and the least restrictive citation.

The survey measures the standard requirements; Legend’s requirements exceed those of the state, so both residences were positioned to be successful. Both have been open since 2014. Ortega obtained the goal of zero deficiencies once before.

Each Legend Senior Living residence conducts its business to be survey-ready every day. “We accomplish this through our quality management and quality review processes to ensure all regulatory items are in compliance,” Thibault said.
