Park West Plaza is a Continuing Care Retirement Community with Independent Living, Assisted Living, Reflections Memory Care, and a Skilled Nursing Home on campus. Independent Living at Park West Plaza
Independent Living at Park West Plaza provides a small and intimate home-like setting within a full continuum of care. Each cottage faces a beautiful courtyard and are all located on ground level.
Independent Living at Park West Plaza includes:
- Lunch meal daily with choice of entrée's
- All utilities paid - except telephone
- Free extended cable television
- Washer and dryer furnished in every apartment
- Individual climate control in each apartment
- Furnished kitchen with "full" size refrigerator, oven and range, sink, garbage disposal, and dishwasher
- One carport reserved for each individual
- Daily life enrichment and exercise programs
- Weekly housekeeping and linen service
- Private covered patios
- Scheduled transportation
- Clubhouse for private parties, card clubs and private family dining
- Handicap accessible bathroom
- 24-hour emergency service
Assisted Living at Park West Plaza
At Park West Plaza, we recognize that each person has individual needs and preferences. With this in mind, we offer each resident a flexible service combination we call Tailored Services. The combination of the Base Monthly Service and the Tailored Services designed for you result in your monthly Tailored Pricing. This unique plan provides you with the most individual personal services in the most cost effective manner.
The Base Service Plan Includes:
- Certified nursing staff on duty 24 hours/day
- Emergency Response System
- Three meals per day and snacks
- All utilities - except telephone
- Individual climate control in each apartment
- Free extended cable TV
- Furnished kitchen with "Full" size refrigerator, oven and range, sink and garbage disposal
- Handicap accessible bathroom
- Daily life enrichment and exercise programs
- Weekly housekeeping and linen services
- Scheduled transportation available for shopping
- Common areas for social gatherings
- Storm shelter and worship area
- Smoke detector and sprinkler system in each apartment

Reflections at Park West Plaza
Reflections at Park West Plaza is a six bedroom home designed to incorporate the most up-to-date research related to memory care. This modern design features attractive lighting and color palette, beautiful furnishings, lake and creek side views, daily life enrichment program and professional staff.

- Beautiful creek side views
- Medication Management
- Private and Companion suites
- Family style dining with menu choices, therapeutic diets and snacks throughout the day
